The format of your logo will depend on the type of organisation you are.

The drop-down menu in the form below contains the range of NHS organisations that we can provide artwork for. Please select the appropriate entry for your organisation, complete the relevant fields and email us your form.

Please note that NHS logotypes are only issued to NHS organisations and only once, contact your communications department in the first instance to access a copy of your original logo before completing the form below.

We provide you with this artwork free of charge, at our own cost, so please store it in a safe place, and let other members of your organisation know where the artwork is kept.
